Key terms

CMS-855A

CMS-855A is Medicare's enrollment application for institutional providers, including home health agencies, hospices and skilled nursing facilities. It reports provider and ownership information for the relevant enrollment action. Submitting it does not establish approval, certification or permission to transfer billing.

Key takeaways

  • Confirm that CMS-855A applies to the provider type.
  • Match legal identity and ownership records before submission.
  • Track the application and final decision as separate records.

Start with the reason for filing

The form serves several enrollment actions. Those include an initial enrollment, revalidation and reported changes. Read the current instructions before choosing an action; a prior seller's completed form may describe a different event.

The linked CMS form was revision 09/24 when checked on September 8, 2026. Its instructions identify the institutional provider types and explain both paper and PECOS submissions. This definition explains the document's role, rather than supplying answers to its fields. (Source: CMS-855A, printed pages 1–3)

A fictional application-status file

Assume a buyer is reviewing a fictional provider's file through approved access. No real identifier or personal record appears below.

Fictional file itemReview questionUnsupported conclusion to avoid
Legal entity recordDoes the name match the applicant?Similar trade names prove identity
NPI recordDoes it identify the correct provider?A number proves enrollment approval
Submission receiptWhich action and date were submitted?Receipt means the transaction is approved
Final decisionWhich entity and action does it cover? An old decision covers a new transaction

A mismatch is an issue to resolve, not a field to fill from a guess. Record the source of each answer and keep the filed version with its supporting documents.

Connect the form with the agreement

The Medicare certification definition separates survey and enrollment decisions. The provider-agreement definition addresses a different document.

For an ownership change, read the CHOW guide alongside the application instructions. Agreement assignment and outstanding obligations require their own review. An enrollment form does not rewrite the purchase agreement or eliminate the seller's history.

Review current restrictions before planning a filing

A home health or hospice transaction also needs a 36-month-rule review and a check of the current enrollment moratorium. Do not assume an initial application is available simply because a form exists. Assign the open filing questions to counsel and the MAC before making the closing schedule depend on them.

Read the Medicare number handoff question when a buyer asks to keep using the seller’s number. Its three-task example shows why identifier continuity, a filing receipt and a billing-service contract answer different questions.

Frequently asked questions

Can the provider use PECOS instead of a paper form?

CMS describes online PECOS and the paper CMS-855A as enrollment routes for institutional providers. Use the current instructions for the provider type and action. An online submission still needs the required information, signatures and supporting records; the platform is not an approval.

Does every ownership change use the same application category?

No. The instructions distinguish CHOW, acquisition or merger, consolidation and other changes of information. Ask the MAC or CMS about the correct treatment when it is unclear. The words used in the purchase contract do not settle the Medicare category.

Is an NPI the same thing as an approved enrollment?

No. The National Provider Identifier identifies a provider; enrollment has its own process and decision. Check that the legal business name, tax identifier and NPI are consistent with the proper records. The presence of a number does not establish current billing rights.

Will filing the form guarantee the planned closing date?

No. Review the required action, supporting documents, outstanding requests and applicable rules with the transaction team. The form does not promise a processing time. The funding and operating plan should reflect the actual decision status rather than an assumed approval date.
